INTERVENTIONS

Directions:

  1. Click on a link below to open form. When asked if you want to open or save, click save and then navigate to where you want to save form. Give the file a relevant name and then click save.
  2. Find where you saved it and click to open the document. Form should open in Microsoft Word or the Open Office Writer.
  3. Once the document opens, click on File, Save As, navigate to where you want to save, type a relevant file name (example: Tier One Documentation - John Doe), change the file type to Rich Text Format, and click save.
  4. Fill out the form using the clickable fields that are created in the document.
  5. When you have completed the form, click to save the document again.
  6. Prepare an e-mail to Cynthia Venson (cynthia.venson@lamarcountyschools.org), our intervention specialist, with an appropriate subject line and message. Attach the saved form you just completed to this email and click send.
