INTERVENTIONS |
Directions:
- Click on a link below to open form. When asked if you want to open or save, click save and then navigate to where you want to save form. Give the file a relevant name and then click save.
- Find where you saved it and click to open the document. Form should open in Microsoft Word or the Open Office Writer.
- Once the document opens, click on File, Save As, navigate to where you want to save, type a relevant file name (example: Tier One Documentation - John Doe), change the file type to Rich Text Format, and click save.
- Fill out the form using the clickable fields that are created in the document.
- When you have completed the form, click to save the document again.
- Prepare an e-mail to Cynthia Venson (cynthia.venson@lamarcountyschools.org), our intervention specialist, with an appropriate subject line and message. Attach the saved form you just completed to this email and click send.
